- Proof of name change, such as a marriage certificate, birth certificate, or legal name change order. The document must be official and issued by a recognized government authority.

The registry agent will provide you with a temporary interim paper License to use until your new driver’s License arrives in the mail (up to 21 days). Your interim paper License will not have a photograph. You will be able to retain your previous identification for photo ID purpose only until your new valid card arrives. The temporary interim and previous ID must be destroyed.
